For New Patients
When you first call to schedule an appointment, we will take your insurance information and verify it with your insurance carrier prior to your first session. We may also give you a login and password to access our patient portal where you can take assessments, complete forms, and print account statements online.

For your first session, please remember to:
- Bring your insurance cards
- Print, read, complete, sign the new patient forms listed below
- Arrive ½ hour early for reviewing paperwork
